SOCIAL WORKER CARE MANAGER

Providing care for over 48,000 individuals annually, we strive to close the gap in health care services for our community through our multi-service medical and dental clinics, senior long-term care programs, teen parenting and adolescent counseling and support groups, drug treatment and HIV/AIDS medical programs.
AltaMed's Mission: To increase access to comprehensive health and human services to multi-ethnic and Latino medically underserved communities in Southern California.
careers at AltaMedProvides comprehensive case management and outreach services based on a home-visiting model to eligible pregnant or parenting teens, some who have MediCal and some who do not. Will assist teens through counseling and referrals to prevent unplanned pregnancies; assess needs and identify resources available within the community in order to provide teens with services and referrals.
Will develop Individual Service Plan (ISP) for each client.
As a responsible team member will contribute to program growth, and will display commitment to the program and company mission and goals.
Requirements: BS (required) in Social Work or other Social Human Services.
Minimum 1 year in case management work or related experience with adolescents is preferred.
Will consider entry level trainee and appropriate Bachelors degree.
Prefer Bilingual (English/Spanish) due to the ethnicity majority of the case load.
Location: Indiana Site and Long Beach Site.
