Bridge is expanding services and in need of several personable, Mentor/Outreach Counselors.
Mentors act as part of the larger West
Bridge Assertive Community Treatment team.
The purpose of this position is to assist clients in making healthy and supportive community connections which will support sobriety, wellness, and overall health.
As a member of our assertive, multi-disciplinary team, the Outreach Counselor ("Mentor") provides active and ongoing community-based outreach and support to individuals who experience severe and persistent mental illness and substance use disorders in the Manchster, NH area.
The Mentor performs a wide range of support activities including but not limited to: Support the Participant in maintaining a healthy environment; Communicate with treatment team nightly after each shift and attend regular supervision with West
Bridge Staff; Assist in identifying community education needs; build awareness of mental illness, recovery, and substance use disorders; Work schedule will vary to include weeknights and weekend nights; Provide ongoing assessment of mental status and consults with West
Bridge staff; Provide the Participant access to their medications and monitor the Participants' self administration of medications; Assist in building support and relationships in the community; develop strong and healthy contacts in the community, including area Peer Support Centers; Maintains compliance with local, state and federal regulations; Represent West
Bridge Community Services in a healthy and positive manner; Portray a positive role-model in all aspects of care and interactions.
Generally, Mentors work any night of the week from 6 pm to 10 pm, and be able to work at least one weekend out of the month.
100% of the Mentor time is spent in the community with a participant/client.
Mentors work a maximum of 20 hours per week.
Requirements: Mentors must be available to work at least one weekend a month; available to attend mandatory, bi-weekly Tuesday night meetings from 5:30p to 6:30p at our Manchester, NH office.
Mentors need to have their own cell phone, reliable transportation with 100,000/300,000 liability insurance coverage, and ability to send a brief email after each shift.
The ideal candidate will be familiar with treatment relating to individuals experiencing substance abuse and mental health related diagnoses.
