Palliative Care Coordinator Job in Wilmington 28403, North Carolina US
Palliative Care CoordinatorExpanding CCNC Network with mission to improve health outcomes for Medicaid program is looking for Palliative Care Coordinator to work with network staff, primary care practices, local hospitals, hospice, and others on development and implementation of a palliative care initiative to improve quality of care for patients at the end of life.
Desired candidate will have clinical experience with end of life care issues; management experience with designing, developing, and implementing interagency collaborative efforts, ability to work collaboratively with a team, form positive relationships, create and deliver presentations, understand and effectively use data, adapt rapidly to an ever-changing set of priorities, work independently, excellent written and oral communication skills, firm grasp of Microsoft Office products.
Position based out of Wilmington office, requires travel within six-county region (approx 1 hour radius from Wilmington).
Full-time, salary with benefits package.
