Intake Coordinator Job in La Mesa, California US
Location This position is located in La Mesa.
Hours 8 hour shift: 9:00 am - 5:30, hours may depend on evaluation needs.
Qualifications Graduate of an accredited Licensed Vocational Nurse (LVN) School of Nursing required.
Must have 1-2 years home health experience, with some experience in management preferred.
Experience as home health liaison, community outreach coordinator helpful.
Current California Licensed Vocational Nurse licensure required.
Current CPR certification required.
Effective interpersonal communication skills.
Ability to demonstrate compassion and courtesy when dealing with difficult situations.
Capable of working under pressure and time constraints including overtime to complete daily assessments.
Use of sound judgment and tactful approach when dealing with co-workers, staff and the public.
Possess the ability to use and apply clinical knowledge to the referral process.
Must assume a position in the weekend rotations.
Basic knowledge of insurances including Medicare/Medi-Cal and HMO limitations.
Summary Knowledge of overall home health/hospice management including admission criteria, nursing process and insurance limitations.
Organizes and coordinates daily intake/referral processes for the hospice agency to ensure that the initiation of care is timely and according to physician orders and within regulatory guidelines.
Acts as the key linkage between hospital case management services, medical group physicians and office staff, hospice agencies and home health services.
Responsible for the coordination of complex patient issues related to home care services.
Physical Requirements This job requires 5-6 hours of sitting at a desk; able to hear and speak on the phone for several hours per day; must have the ability to move in small spaces; Fluent knowledge of English, both verbal and written, fine motor dexterity; intermediate keyboarding skills.
