Fulltime RN Case Manager Needed Immediately for Home Health Agency RN needed for home visits in Upland and surrounding areas (Monday Friday) Duties: - Will be making visits for the following: Starts of Care, Recertifications, Discharges, Resumptions, and weekly Supervisory visits for LVNs and CHHAs- Will be working closely with the D.PCS and other staff members to ensure all patient and family care needs are being met Qualifications: - Experience with Oasis paperwork preferred- Must have great verbal and written communication skills- Must have at least 1 year experience as a Home Health RN
