Maxim Healthcare Services is seeking a Clinical Case Manager to be responsible for intermittent visits for a geographically assigned caseload of patients.

The Case Manager will be responsible for planning, coordinating, providing and documenting care for designated patients.

Responsibilities and Duties of a Maxim Case Manager Include: Develops and implements a plan of care that meets each patient's specific needsPerforms admission assessmentProvides skilled nursing care per visitRequests modification of orders or additional orderMakes recommendations for continued care or discharge of patientsSupervise Home Health Aides on a bi-weekly basisWill average 5 visits per day or 25 visits per week Maxim Healthcare Services is one of the leading providers of medical staffing, home health and wellness services in the United States.
