Case Manager, RN Job in Belleville 07109, New Jersey US
Coordinates the care and services of Clara Maass Medical Center patient population across the continuum of illness.
Assumes responsibility for utilization management and monitoring process.
Conducts admission and continued stay review on groups of patients, in accordance with the PRO, special negotiated PPO and/or HMO contracts, and on other classifications of patients as designated by the Utilization/Case Management Review Plan.
Coordinates patient care, in conjunction with the medical, nursing and other ancillary staff members.
Identifies trends and patterns of resource use and patient care, and makes recommendations in compliance with established policy and procedure.
Ensures continuity of care for those patients with post-hospital health care needs and maximizes the patient's and family's abilities to meet the continuing care needs within their social and financial framework.
Uses an understanding of principles of growth and development to assess each patient's age specific needs and provide age specific treatment and care.
Registered Nurse with a Baccalaureate Degree from an accredited school or university preferred, or equivalent RN Degree with commensurate clinical experience.
Minimum of 2-3 years experience in a hospital setting.
Minimum of one year experience in Case Management/Utilization Management or Home Care.
