Care Coordinator Job in 05156
HCRSHEALTH CARE & REHABILITATION SERVICES of SOUTHEASTERN VERMONTA Community Mental Health Agency Serving Windham and Windsor CountiesRewarding Opportunity to make a difference -Care Coordinator, Child and Family Support ProgramFull time position (37.
50 hour) Springfield Providing case management and support to children and families through collaboration with the Vermont Department for Children and Families.
Work with biological and foster families in home, office, community and DCF settings.
Provide transportation and coaching for family visitations.
Coordinate services and work in collaboration with other providers.
Candidates should be organized, engaging, supportive and self motivated with excellent communication skills.
Will share 24/7 on-call duties with others on a scheduled rotation.
Bachelor's degree required.
Training in Family Group Conferencing, Family Safety Planning, or Family Time Visit coaching a plus.
