We are seeking a RN certified case manager (CCM) to work on-site at one of our primary providers in Worcester.
FCHP case managers use a multidisciplinary approach to organize, coordinate, monitor, evaluate and modify plans of care pre-admission to post-discharge.
They work independently and collaboratively as a facilitator with other members of the health care team to ensure that members receive quality, cost-appropriate care.
Qualifications: Active, unrestricted licensure as a Registered Nurse in Massachusetts and current Massachusetts driver's license.
Certified in Case Management and BSN preferred.
A minimum of three to five years clinical experience as a Registered Nurse in an inpatient setting required.
One year experience as a case manager in a payor or facility setting highly preferred.
Excellent verbal and written communication skills and computer literacy required.
