Primary Duties/Responsibilities Under supervision, performs a wide variety of clinical and concrete duties to provide assessment/reassessment, intervention and planning to promote continuity of care for post hospital discharge.
Understands the lifecycle changes related to health care with ages spanning from elderly to newborn.
Supervision of interns as assigned.
Minimum Qualifications BSW or MSW, 1 year equivalent experience (internship to be considered) & LSW or CSW required.
Two years healthcare social work & ACSW preferred.
