Overview: The HOMECARE NURSE (RN) provides direct patient care to ensure the effectiveness of the care, treatments and services being provided by the agency, meet the patient's needs Responsibilities: The Visit Nurse provides skilled nursing services to his/her assigned patients, and communicates progress toward goals and/or any changes needed for the plan of care with the Care Team Manager and/or ADCS, who is responsible for the overall case management of the patient.
He/she works under the direct supervision of the Care Team Manager and/or ADCS.
This description is the basis for all nurses who are seeing patients in a Medicare certified agency with Care Teams.
Specialty skills are outlined within the company's competency requirements Qualifications: A Registered Nurse graduated from an approved school of nursing with a minimum of one year of acute care nursing experience required; previous home care or hospice nursing experience preferred.
2. Licensed as a Registered Nurse in the state(s) in which he/she is required to practice.
3. Ability to relate to and treat acutely ill/terminally ill patients/families and their particular problems.
4. Must possess strong clinical assessment, evaluation and patient education skills.
5. Must possess current knowledge of clinical techniques and procedures and be able to effectively perform the duties and tasks outlined in the patient's care plan.
5. Previous experience in venipuncture skills and intravenous therapy administration preferred, and/or specialty experience in area offered by agency/care team.
6. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the patient.
7. Excellent organizational, interpersonal, telephone, communication, and documentation skills.
8. Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff, and interact professionally as a member of a multi-disciplinary team.
9. Must possess a strong commitment to the goals, mission, and philosophy of the organization.
10. Ability to work flexible hours as required to meet identified patient needs.
11. Absolute integrity and the use of good judgment to handle the professional and confidential nature of assignments.
12. Ability to adapt to changing organizational needs.
13. Speaks, writes, reads, and comprehends English.
