Outreach Coordinator - (CG16775)
Job Description: Job Summary: The Perinatal Center Fetal & Infant Mortality Review (FIMR) Outreach Coordinator participates in the implementation of the Healthy Start FIMR Project and works with the hospitals, the coordinator and other staff to successfully complete the project.
Job Qualifications: Education: Bachelors degree, Nursing or Social Work required Masters degree, Nursing or Social Work preferred License: Current RN Illinois License or current Social Work Illinois License required Other Transportation for hospital and/or home visits required Valid Illinois drivers license required Experience: Minimum requirement of five years of clinical practice as RN in obstetrics and /or pediatrics or minimum of five years experience as a social worker in the community working within obstetrics and/or pediatrics Community Health or Public Health experience required Requires basic abilities to use word processing software to produce appropriate routine correspondence Requires good interpersonal and communication skills Incumbent must have the proven ability to function with a high degree of autonomy within a complex organization and a network of hospitals We are proud to be an EEO/AA employer M/F/D/V.
