Department: Adult Psychiatry Schedule: Part time regular, 20 hrs week, Mon-Fri, varies *hours may vary to meet the needs of the department Education: Master's Degree, from an accredited graduate program, in Social Work, Marriage and Family Therapy, Counseling, Counseling Psychology, or Psychology.
License: Current valid license to practice in the state of California as a Clinical Social Worker (LCSW) or Marriage and Family Therapist (LMFT), California driver's license.
Will consider a LCSW or LMFT.
Work Experience: Experience and proficiency in clinical assessment and long term case management for adults with severe and persistent mental disorders, especially mood disorders, chronic psychoses, and co-occurring substance abuse; knowledgeable application of laws and regulations governing confidentiality, voluntary and involuntary psychiatric care, scope of practice, and related matters; rapid assessment and intervention with patients in acute crisis; treatment and education in group as well as individual formats; building alliances with allied caregivers, community agencies, and, when appropriate, with family members to ensure continuity of care and to educate as regarding nature and course of the illness and essentials of the treatment plan; accommodating the needs of patients from diverse cultural backgrounds; (bilingual English/Spanish capability highly preferred), collaborating with a multidisciplinary treatment team; working in a labor-management partnership environment.
Duties: Provider will be a member of the Adult service team, will carry a significant ongoing caseload of adult patients who suffer from mental illness which is not responsive to brief therapy.
Carries primary responsibility for ensuring continuity of care as treatment moves to and from higher levels of care, especially Intensive Outpatient Programs and hospitals, keeping all allied providers aware of patient's location, clinical status, and treatment requirements.
May provide IOP services directly as well.
Primary role is to perform ongoing clinical case management to maintain the stability of these patients, providing appropriate clinical services through a combination of direct treatment, referral, follow-up, and program development.
Charts member's treatment and progress in accordance with state and NCQA regulations and with accepted community standards.
Utilizes resources of public and private agencies and community organizations to meet the needs of the member's treatment.
May be involved in the triage and disposition of members who present as urgent cases; and will participate in hospital after-hours on-call rotation.
Collaborates with physicians in screening and evaluating patients for psychotropic medications, within the scope of practice.
Collaborates with treating primary care providers and other departments such as the ED, hospital, mental health agencies, and other community resources.
Must treat external and internal customers in a respectful, friendly, helpful, and professional manner at all times.
Must adhere to the service expectations of the department and facility.
