What to put into references on my application form?

I can't get in contact with my previous employer. The number I had for them no longer works and my manager at the time does not work there any more. I now also live about 240 miles away...
This reference is the only one relevant to the job I am applying for (both are bar staff)
Should I put references for the two previous jobs, both of which were call centres?
Or just give them all information possible?

Well if they ask you to put most recent employer, well you need to include that unless you can give a good reason not too. You say the number you have no longer works... have your tried directory enquiries to get a new number, have you Googled the company / bar / whatever to try and get contact details.. At least then you say you tried - and then its hardly your fault that anyone that knew you no longer works there. |

Meh any reference will do for a job like bar staff. they will just want to get to know your character so the call centre references should be able to provide that. |

I always used to use my old school since my teacher used to really like me. Gets a bit weird to do that once you go past 20 though so I stopped. I suggest that you put down that you worked in a bar before in the previous experience section of the application form and include the other two jobs as your references. References do not necessarily have to be relevant, they just want to know that you will be a good worker. |
