Shall I leave it blank or put "Not Applicable" on the job application?

Ok, so I apply for a job and the application ask for the following things. Can u check the one in the parentheses to see if it's ok to put that, if not plz tell me what to put, thx!!! Oh, shall I put "Not Applicable" or leave it blank, see below.

* Supervisor's Name/Title: (Not Applicable)

* Supervisor's Phone: (Not Applicable)

* Reason for Leaving: (I am still working for the above employer. Employer's Phone: (777) 777-7777)

The reason that I put that answers is b/c my only job so far is just as a caregiver. I am currently working at the client's house. There is no company, supervisor at all.

In the sections

* Supervisor's Name/Title * Supervisor's Phone i would just put the details in then add a small note explaining that you work for them privately.
