Does anyone know whether this is supposed to be undertaken anonymously ?

I only ask as a recent job application form included an additional "Equal opportunities monitoring form" but in addition to ethnic origin & disabilities etc you were required to write your name and job applied for on the form. I had always assumed this was purely statistical data supposedly acquired anonymously and my experience with previous job applications (over the past 30 years) would tend to support that.

Does any Human Resources person or Legal eagle know what the Law is regarding this (in the UK) ?

There is no legal requirement to complete this monitoring form when you apply for a job. Normally they ask for the job reference so that they can analyse the data for each job as well as recruitment trends overall. There is usually no reason to ask for your name but it is not illegal to do so. Some employers now ask for your name on the equal opps form but the application form which the assessment panel see has only your initial. That means when selecting candidates for interview there is no gender discrimination. If you don't want to include name then there is no reason to.
