All you need to bring is yourself! Job Description Description Responsible for care coordination, discharge planning, utilization review, interdisciplinary plan of care, coordinating complex case review for an Acute Medical population.

Qualifications Must be a Registered Nurse with an active RN license in the State of Alabama or the ability to obtain.

Two to five years acute care clinical experience required.

Current Case Management experience preferred but not required.
