Clinical Manager
Sutter VNA & Hospice is one of the largest, not-for-profit home health and hospice agencies in Northern California.
What began in 1906 as the selfless act of volunteer nurses caring for children injured in the 1906 earthquake has evolved into today's Sutter VNA & Hospice.
Sutter VNA & Hospice enables patients to recover or live with illness in the comfort and dignity of home, surrounded by the people and things they love.
Our home care programs focus on helping patients by administering care and teaching patients to become more independent in managing their illness.
Our hospice program cares for the patient and the family, focusing on care, not cure, maximizing the patient's comfort and dignity and also addressing the emotional and spiritual needs that arise when a patient is dying.
Sutter VNA & Hospice is accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).
Sutter VNA & Hospice is one of Northern California's largest not-for-profit, community based home care and hospice agencies, providing high quality health care in patients' homes.
Services include visiting nurses, rehabilitation therapists, medical social workers; home infusion therapy; home health aides, hospice care; bereavement programs; and community health clinics.
Sutter VNA & Hospice serves ten Northern California counties with local offices in Emeryville, Concord, San Leandro, San Mateo, Santa Rosa, Sacramento, Tracy, and Roseville, and manages home health and hospice services in Marin County and San Francisco.
The Clinical Mgr (CM) assists the Director of Patient Care Services (DPS) in the planning, organizing, and directing of patient care services in accordance with current standards and regulations.
This position directly supervises the team(s)providing direct care for patients.
The CM directs patient assignments, staff scheduling & patient visit volume distributions, & assists in meeting financial targets by managizing utilization, productivity, personnel and supplies.
Participates as necessary in team meetings and represents clinical staff on appropriate committees.
The CM assists with staff development, orientation, interdisciplinary team mtgs, inservice training & evaluation of personnel for all disciplines involved in patient care services.
The CM also provides direct patient care in times of high census and/or for emergent need.
Other managerial tasks and projects as assigned by DPS. Graduate from an accredited RN program.
Current California RN license.
Previous clinical experience in a home health or hospice agency preferred; Minimum one year previous supervisory experience in a health care setting required.
Must have a minimum of two years experience in own discipline within the last five years.
Shall have sufficient background knowledge and expertise in clinical decision-making for the patient populations assigned, to meet the needs of the patients and to contribute to Quality Management review and evaluation.
